At Mani Physiotherapy, we accept Chronic Disease Management (CDM) plans (formally known as Enhance Primary Care or EPC plan).
This will allow your appointment to be rebated through Medicare, lowering your out-of-pocket cost. Please note we do not bulk bill, and a small gap fee will be required.
Firstly, visit your GP to see if you are eligible (most chronic health conditions are) and ask your GP for a Chronic Disease Management plan and referral to Mani Physiotherapy. This can be sent directly to us via fax and we will call you to arrange an appointment. Alternatively, give us a call when you have your referral to make a booking.
